    A 13-year-old boy with Cushing syndrome presented with symptoms including moon face, weight gain, and growth retardation over a year. Elevated serum cortisol levels and unsuppressed urine free cortisol confirmed the diagnosis. Bilateral inferior petrosal sinus sampling localized the excess cortisol to the pituitary gland. Physical examination revealed characteristic features such as bilateral supraclavicular fat pads, a back hump, wide purplish striae, and bilateral scrotal masses. Hormonal evaluation showed low levels of follicle-stimulating hormone, luteinizing hormone, and testosterone, with a bone age of a 10-year-old. The diagnosis was confirmed as Cushing syndrome.
