An 11-year-old girl initially misdiagnosed with tinea amiantacea was later correctly identified with tinea capitis favosa caused by Trichophyton schoenleinii. Despite treatment with griseofulvin and ketoconazole, she developed permanent alopecia. This case underscored the critical need for mycological examination in diagnosing diffuse scalp scaling to avoid irreversible hair loss.
