TLDR The TAP flap is effective for treating armpit scars from burns, and tissue-engineered templates with hair follicles can help treat scalp burn alopecia.
The document reports on two separate cases. The first case describes the use of a thoracodorsal artery perforator (TAP) flap in a 47-year-old male patient with an armpit burn that had previously undergone a skin graft. The TAP flap procedure was performed to remove the graft and skin retraction, which improved the patient's shoulder movements and reduced pain. The authors concluded that the TAP flap is a good option for treating armpit scar retractions after deep burns, emphasizing the importance of understanding the anatomy of the region for successful flap implementation. The second case discusses the treatment of postburn alopecia in a child with a full-thickness flame burn of the scalp. The treatment involved scar resection followed by immediate reconstruction using a tissue-engineered dermal template (Integra®) and implantation of autogenous hair follicles. The hair follicles integrated within 21 days, and neoepidermis formation was observed around the hair grafts. Immunohistochemistry studies indicated the presence of cytokeratins and neovascularization markers, although the authors noted that further studies are needed to confirm if the new epidermis originated from follicular bud stem cells.
