Testosterone Enantate Induced Androgenetic Alopecia in a 46XX True Hermaphrodite

TLDR A man developed hair loss from testosterone treatment but improved with additional medication.
An adult male phenotype 46XX true hermaphrodite developed iatrogenic androgenetic alopecia (AGA) after being treated with testosterone enantate for testosterone replacement therapy. The patient, who had a history of progressive hair loss starting at age 18, had previously undergone bilateral orchiectomy and testis prosthesis insertion at age 16, with testosterone enantate therapy initiated thereafter. The hair loss symptoms appeared after 3 years of testosterone treatment, and physical examination showed frontotemporal recession and Norwood-Hamilton type III vertex alopecia. Microscopic analysis confirmed the diagnosis of iatrogenic AGA. The patient was then treated with finasteride in addition to the ongoing testosterone therapy, which resulted in signs of hair regrowth and stabilization of hair loss after 4 months.
