A 4-year-old girl presented with symptoms including fatigue, alopecia, and weight loss, which were linked to a large gastric trichobezoar, a condition known as Rapunzel syndrome. This rare condition involves a hairball extending from the stomach into the small bowel, often leading to anemia and potential complications like perforation. The girl's trichobezoar was surgically removed, and she recovered well postoperatively. The case highlighted the importance of considering psychiatric evaluation to address underlying stressors and prevent recurrence. Rapunzel syndrome is rare, with about 40 pediatric cases reported, and requires a high index of suspicion for diagnosis.
