A 29-year-old female with resistant polycystic ovary syndrome (PCOS) experienced persistent symptoms despite initial medical treatments. Elevated testosterone levels and bilateral polycystic ovaries were confirmed through laboratory tests and imaging. The patient underwent a right unilateral oophorectomy, which led to significant improvement in her symptoms, including reduced hirsutism, regularized menstrual cycles, and normalized testosterone levels. This case demonstrated that unilateral oophorectomy could be an effective second-line treatment for resistant PCOS when first-line medical therapies fail.
