TLDR Stopping finasteride improves sperm quality but not semen volume in young men.
In a retrospective study of 20 young male patients with finasteride-associated infertility, it was found that discontinuation of finasteride led to improvements in sperm concentration and total motility, but semen volume did not change over a 24-week period. Despite finasteride's known effect of reducing prostate volume, discontinuation did not reverse this reduction. The study concluded that finasteride's impact on semen and prostate volume might be due to androgen deprivation, which affects the structure and function of male sex organs. These findings highlight the need for careful consideration when prescribing finasteride or similar drugs to young men who wish to maintain fertility.
