TLDR Early recognition and treatment of scalp demodicosis can prevent misdiagnosis and effectively resolve symptoms.
This case report highlights a rare instance of scalp demodicosis in a 30-year-old woman with a history of androgenic alopecia, who presented with intense scalp pruritus and hair shedding. Trichoscopic evaluation revealed multiple Demodex tails, confirming the diagnosis. The patient was successfully treated with topical metronidazole, resulting in complete resolution of symptoms. The report emphasizes the importance of early recognition of scalp demodicosis through trichoscopy, as it can prevent misdiagnosis and unnecessary invasive procedures. The study underscores that Demodex mite infestation, though often overlooked, can cause scalp folliculitis, itching, and hair shedding, and highlights the effectiveness of timely acaricidal therapy.
