    TLDR A man had a severe allergic reaction to sulfasalazine, with symptoms improving after treatment and follow-up.
    In 2011, a 27-year-old man developed a hypersensitivity syndrome after being treated with sulfasalazine for chondromalacia patella. He presented with symptoms including fever, maculopapular exanthema, dyspepsia, vomiting, and pharyngoamygdalitis, which did not improve with cefuroxime and acetaminophen. He also had an enlarged liver and spleen, jaundiced mucosa, distal purpuric edema, adenopathies, and leucocytosis. A skin biopsy indicated necrotic keratinocytes, and CT scans showed multiple bilateral adenopathies. The reactivation of human herpes virus 6 was detected, and he tested positive for anticardiolipin antibodies. Despite initial treatment with methylprednisolone, he experienced a relapse, requiring a more gradual dose reduction. At a 12-week follow-up, he had telogen effluvium but no other symptoms, and while his anticardiolipin antibody levels were normal, his thyroid screening was not.
