TLDR Congenital triangular alopecia and vitiligo can occur together, but treatment for one may not affect the other.
A 12-year-old male presented with congenital triangular alopecia (CTA) and newly developed vitiligo. CTA, a non-scarring alopecia, was diagnosed based on clinical and dermoscopic findings, showing a triangular patch of hair loss with normal follicular openings and vellus hair. The patient also had depigmented patches on his foot, confirmed as vitiligo. Treatment with tofacitinib improved the vitiligo but not the alopecia. This case highlights the rare co-occurrence of CTA and vitiligo, with no previous reports of such an association. The condition remains benign and asymptomatic, with no definitive treatment for CTA.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.
