TLDR COVID-19 may lead to severe skin necrosis without clear underlying causes, as seen in a diabetic patient who required leg amputation.
This case report details a 68-year-old male with controlled insulin-dependent diabetes who experienced severe skin and soft tissue necrosis on his lower limbs during a mild COVID-19 infection. Despite no signs of vasculitis or thrombotic occlusion and negative blood cultures, the patient's condition worsened, resulting in osteomyelitis and necessitating leg amputation. Treatment included antibiotics, corticosteroids, and advanced wound care. The report suggests that COVID-19 may cause severe vascular-related skin necrosis, though the exact mechanisms are unknown. The authors propose that the viral infection initiated the necrosis, with bacterial superinfection aggravating the condition.
