TLDR Paraproteinemic keratopathy can show eye symptoms before other signs of disease, needing careful treatment and long-term follow-up.
Paraproteinemic keratopathy, linked to conditions like multiple myeloma and MGUS, can present ocular symptoms that may precede systemic ones. This study discusses 3 new cases and 2 follow-ups, showing varied treatment responses including chemotherapy, superficial keratectomy, and PKP. One patient had long-term crystal resolution post-chemotherapy and ASCT despite a relapse. The study emphasizes the need for long-term follow-up and further research to determine optimal treatments, as ocular symptoms alone do not warrant systemic therapy. Treatment decisions should consider chemotherapy's side effects and patient preferences, with a call for prospective multicentre studies and collaboration among healthcare providers.
