This study on alopecia areata (AA) involved 60 patients and highlighted the importance of trichoscopy in diagnosing AA by identifying specific trichoscopic features such as black dots (95%), yellow dots (77.5%), and vellus hair (67.5%). Notably, trichoptilosis, a longitudinal split in the hair shaft, was observed in 20% of cases, suggesting an overlap with features of other hair disorders like trichotillomania. The findings indicate that the loss of immune privilege in AA may lead to hair shaft abnormalities, necessitating a reevaluation of differential diagnoses between AA and other hair disorders. Further research is needed to explore the mechanisms and clinical implications of these findings.
