The study identifies unique circulating microRNA (miRNA) signatures that distinguish severe alopecia areata (AA) from other inflammatory skin diseases, using a cohort of 110 participants. Researchers found 19 miRNAs significantly downregulated in severe AA, with 10 showing high specificity for AA. A machine learning model using the top four miRNAs achieved a high classification accuracy (AUC = 0.94). Pathway enrichment analysis highlighted dysregulated immune and metabolic pathways, suggesting potential therapeutic targets like kinase inhibitors. The study underscores the potential of miRNAs as biomarkers and suggests new treatment avenues for AA, although it notes limitations such as small sample size and lack of longitudinal data.
