Trapped in the Hypoglycemia Loop: A Case Study of Malignant Insulinoma

TLDR Early advanced therapies are crucial for better survival in aggressive insulinoma cases.
This case study highlights the aggressive nature and management challenges of metastatic insulinomas, as demonstrated by a 21-year-old male with recurrent hypoglycemic episodes due to a high-grade neuroendocrine tumor. Despite a multimodal treatment approach, including medical management, radiofrequency ablation, and transarterial embolization, the patient's condition rapidly progressed, leading to severe complications and ultimately death. The study emphasizes the importance of early systemic chemotherapy and advanced therapies like everolimus and PRRT in improving survival outcomes for patients with malignant insulinomas.
