Trapped in the Hypoglycemia Loop: A Case Study of Malignant Insulinoma

Zhang Lian, Chin Voon Tong, Zanariah Hussein
This case study of a 21-year-old male with malignant insulinoma highlights the aggressive nature and management complexities of the condition. Despite a multimodal treatment approach, including medical management, radiofrequency ablation, and transarterial embolization, the patient's condition rapidly progressed, leading to severe hypoglycemic episodes, recurrent sepsis, multi-organ dysfunction, and ultimately death. The study underscores the importance of early administration of systemic chemotherapy and advanced therapies such as everolimus and peptide receptor radionuclide therapy (PRRT) in improving survival outcomes for patients with high-grade insulinomas.
