TLDR Down Syndrome patients with Alopecia Areata often have hypothyroidism and specific immune abnormalities.
The study evaluated the immunological profiles of 83 Down Syndrome (DS) patients, 21 with Alopecia Areata (AA) and 62 without. The average age was similar between groups, and 94.7% had free trisomy. Hypothyroidism was more common in DS patients with AA. Hemogram abnormalities were frequent, with elevated Blood Sedimentation Rate (BSR) in 71.1% and decreased CD4 levels. Immunoglobulin levels were mostly normal, but Complement C4 and C3 were decreased in 67.4% and 9.6% of patients, respectively. The presence of antiperoxidase antibody was significant in DS patients with AA, potentially linked to hypothyroidism. The study suggested further research with larger samples and HLA testing to better understand AA in DS.

In this conversation, 4990 discussed various treatments for hair loss, including oral minoxidil, PRP, transplan, Jak inhibitors, Dutasteride, Finasteride, Olumiant, Ketoconazole, RU58841, microneedling, baricitinib, and CCCA. They recommended scalp biopsies in unclear cases of DUPA, twice weekly to twice daily shampooing for topical minoxidil users, and two sessions spaced one month apart with follow up at month three to determine the effectiveness of PRP treatment.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.

The conversation discusses skepticism around claims that dutasteride worsens hair loss, with some users suggesting misinformation or panic during shedding as reasons for such claims. One user shared their negative experience with dutasteride, including increased sebum production and hair loss, and is now considering a higher dose of oral minoxidil after stopping dutasteride.
