TLDR The woman's forehead lesion was caused by ointment use and resolved with treatment.
A 79-year-old female patient presented with a hyperpigmented, crusting, and itching lesion on her forehead that resembled dyskeratosis follicularis Darier. She had no history of skin disease. Examination and biopsy revealed epidermal acanthosis and enlarged follicle ostia with foreign material. The patient had been using an ointment and had a cosmetic treatment with an unspecified powder. The diagnosis of pomade crust was confirmed. Mechanical enucleation of the follicular material and treatment with metronidazole cream led to complete resolution of the lesions.
