Antinuclear antibody (ANA)-negative lupus nephritis (LN) is rare but significant, as seen in a 25-year-old female with symptoms of systemic lupus erythematosus (SLE) but negative ANA tests. Despite initial diagnostic challenges, clinical judgment led to a diagnosis of LN, and prompt treatment resulted in significant improvement. A subsequent renal biopsy confirmed diffuse class IV LN with full-house nephropathy. This case emphasizes the need for a comprehensive diagnostic approach beyond ANA positivity, incorporating clinical features and immunological markers, to ensure timely and life-saving intervention in ANA-negative SLE patients.
