TLDR A rare case of scleredema in a diabetic woman showed loss of sweat glands, causing heat strokes, with treatment only slightly improving symptoms.
The document described a unique case of scleredema adultorum in a 50-year-old Taiwanese woman with a long-standing history of type 2 diabetes mellitus, marked by the rare loss of eccrine glands, resulting in anhidrosis and frequent heat strokes. Treatment with oral allopurinol for 8 months did not resolve the anhidrosis, though there was a slight reduction in erythema and induration. This case underscored the slow progression and persistence of scleredema associated with diabetes, contrasting with infection-related cases. The loss of eccrine glands was a novel finding not seen in 22 other cases reviewed, with its cause remaining unknown and no evidence of autoimmune diseases.
