TLDR Captopril reduces heart mass, while minoxidil has opposite effects and side effects.
This study compared the effects of captopril and minoxidil on left ventricular hypertrophy in resistant hypertensive patients over a 6-month period. The results showed that captopril-based triple therapy and minoxidil-based triple therapy had opposite effects on the heart despite comparable lowering of blood pressure. Captopril was found to be statistically in favor because it resulted in a mean decrease of 42 g in the left ventricular mass index, which exceeds the 7% decrease required. However, minoxidil caused hypertrichosis and ankle edema in some patients. The study suggests that different mechanisms may underlie the effects of these drugs on left ventricular hypertrophy.
