TLDR A rare scalp condition, cutis verticis gyrata, was found in a woman with primary scarring alopecia.
A 39-year-old female with a history of primary scarring alopecia presented with scalp tenderness, pruritus, and vertex hair loss, which began 13 years prior. She developed scalp undulations, diagnosed as cutis verticis gyrata (CVG), a rare condition more common in men. Examination revealed deep furrows and follicular dropout. A biopsy confirmed CVG and cicatricial alopecia. Treatment included topical clobetasol, intralesional triamcinolone, and oral minoxidil. The case highlights the rare association of primary scarring alopecia with CVG, emphasizing the need for further research into the pathogenesis and potential genetic links between these conditions.
