Coronal Approach for Rehabilitative Orbital Decompression in Graves' Ophthalmopathy

    TLDR The coronal approach is effective and safe for reducing eye bulging in Graves' eye disease.
    The retrospective study evaluated 125 patients with Graves' ophthalmopathy who underwent a three-wall orbital decompression using the coronal approach, finding it to be an effective and safe method for reducing proptosis. The mean preoperative proptosis was 22.43 mm, with an average reduction of 4.34 mm post-surgery. Patients with higher preoperative proptosis saw greater reductions. New postoperative diplopia occurred in 3.2% of patients, while 4% with preoperative motility disturbances experienced improvements. The study highlighted the importance of individualized operation indications and proptosis reduction goals, considering both Hertel measurements and historical photographic evidence. The coronal approach was particularly beneficial for patients with hair to hide the surgical scar, and the study noted a higher number of female participants due to the prevalence of the condition in women and the approach's suitability for those without male pattern baldness.
