Contraceptive Care of Androgenized Women with a Low-Dose Cyproterone Acetate Monophasic Preparation

    L. Moltz, J. Meckies, J. Hammerstein
TLDR Diane is effective for treating acne, seborrhoea, androgenic alopecia, and mild hirsutism in women.
101 hirsute women were treated with Diane®, a combination of 2 mg cyproterone acetate and 50 μg ethinyl oestradiol per capsule, from the fifth to twenty-fifth day during a total of 1105 cycles. The treatment was effective in 96% of women with acne, 84% with androgenic alopecia, 79% with seborrhoea, and 50% with mild or moderate hirsutism. The effectiveness for hirsutism and alopecia depended on the severity and duration of the symptoms. There was no correlation between androgen levels and clinical results. Side effects were similar to other contraceptive one-phase preparations. No pregnancies occurred. Diane was recommended as the preferred hormonal contraceptive for patients with acne, seborrhoea, androgenic alopecia, or mild hirsutism, and suitable for follow-up treatment after high-dose cyproterone acetate therapy for severe hirsutism.

A 20-year-old started taking finasteride for hair loss at 18, saw improvement, but developed erectile dysfunction (ED) after taking accutane. Despite stopping both medications, ED persisted, and hair loss worsened. Advice given includes considering tadalafil for ED, resuming finasteride, using minoxidil, and addressing psychological factors through positive thinking and lifestyle changes.
