TLDR Scalp biopsies are essential for diagnosing congenital alopecia areata.
This document discusses a case of congenital alopecia areata, a rare and difficult-to-diagnose condition. Despite the reluctance of patients and guardians to consent to invasive skin biopsies due to concerns about scarring, the case highlights the necessity of a biopsy for a definitive diagnosis. Clinical and trichoscopic findings alone were suggestive but insufficient. The report emphasizes that, according to a literature review, no non-invasive and reliable diagnostic method currently exists for congenital alopecia areata, underscoring the continued importance of scalp biopsies in diagnosis.
