Acute Chylous Peritonitis Due to Idiopathic Pancreatitis Mimicking Acute Appendicitis

TLDR A man thought to have appendicitis actually had a rare pancreas-related condition.
A 23-year-old man presented with symptoms mimicking acute appendicitis, but was diagnosed with acute chylous peritonitis due to idiopathic pancreatitis. This rare condition involves the accumulation of chyle in the peritoneal space and is often diagnosed intraoperatively, as clinical and imaging findings are nonspecific. During laparoscopy, a milky-like chylous effusion was discovered, and the diagnosis was confirmed through biochemical analysis of the peritoneal fluid. The treatment involved peritoneal lavage and the insertion of drains.
