When Catastrophic Antiphospholipid Syndrome Meets Acquired Hemophilia A: A Diagnostic and Management Challenge in Newly Diagnosed Systemic Lupus Erythematosus

This case report details the complex management of a 30-year-old man with systemic lupus erythematosus (SLE) who developed both catastrophic antiphospholipid syndrome (CAPS) and acquired haemophilia A (AHA). CAPS, characterized by widespread thrombosis, and AHA, marked by bleeding due to factor VIII autoantibodies, presented a treatment challenge due to their opposing therapeutic needs. The patient, who also exhibited alopecia, was successfully treated with aggressive immunosuppressive therapy, plasma exchange, and intravenous immunoglobulin, which eradicated autoantibodies and normalized factor VIII levels. The study underscores the necessity of early recognition and a multidisciplinary approach to manage such complex autoimmune conditions effectively, highlighting the importance of rapid intervention to prevent life-threatening complications.
