This case report discusses the successful treatment of a young diabetic female patient using bilateral internal mammary artery (BIMA) grafting for coronary artery bypass grafting (CABG) at Square Hospitals Ltd, Dhaka. Despite concerns about potential sternal complications in women undergoing total arterial grafts, the procedure was completed without any complications, and the patient remains symptom-free after months of follow-up. This is noted as the first reported case of BIMA grafting in a female patient at the institute, highlighting that with careful preparation, BIMA grafting can be safely performed in females, considering potential sternal healing issues.
