TLDR Alopecic and aseptic nodules of the scalp are rare, treatable, and often resolve with doxycycline or on their own.
Alopecic and aseptic nodules of the scalp (AANS) was a rare, likely underdiagnosed, nonscarring alopecia presenting with dome-shaped nodules, primarily in young males. The study included a case report of a 39-year-old man and a systematic review of 71 patients from 15 articles. The nodules were typically firm, located on the vertex or occiput, and often asymptomatic. Histopathology showed mixed inflammatory infiltrates without microbial colonization. Doxycycline (100-200 mg/day for 8-12 weeks) was the most common treatment, leading to complete remission with hair regrowth in 89% of cases. The prognosis was generally good, with spontaneous resolution also observed.
