Acute Vision Loss Due to Retinal Arteriolar Occlusion After Forehead Injection of Triamcinolone Acetonide

    A 33-year-old female with alopecia areata experienced acute vision loss following a forehead injection of 15 mg triamcinolone acetonide. Ocular examinations revealed significant retinal damage, including retinal arteriolar filling defects and decreased vascular density. Immediate treatment with urokinase arterial thrombolytic therapy, ocular massage, and various medications led to partial recovery of vision from inexact hand movement to 40/200 over one month. The case highlights the risk of retinal artery occlusion (RAO) from facial steroid injections and underscores the importance of prompt diagnosis and treatment to mitigate vision loss.
