Part I: Accuracy of Teledermatology in Inflammatory Dermatoses

    TLDR Teledermatology is highly accurate for diagnosing inflammatory skin conditions.
    The study, conducted in São Paulo, Brazil, assessed the accuracy of teledermatology in diagnosing inflammatory dermatoses in a retrospective cohort of 30,976 patients with 55,012 lesions. Focusing on the 20 most frequent inflammatory dermatoses, the study included 739 patients and lesions. The findings showed a high level of agreement between teledermatologists and in-person dermatologists, with a complete agreement rate of 78% and a substantial Cohen's kappa of 0.743. The combined total and partial agreement rate was 86%, indicating almost perfect agreement. The accuracy varied by disease, with some diseases showing over 90% agreement and others less than 60%. Despite potential biases and missing data, the study, which is one of the largest of its kind, supports the viability of store-and-forward teledermatology for patient care in diagnosing inflammatory dermatoses.
