A 70-Year-Old Woman Presenting With Diarrhea and In-Hospital Cardiac Arrest

    A 70-year-old woman with a history of asthma, renal transplant, and atrial fibrillation presented with symptoms including diarrhea, dyspnea, weight loss, fatigue, and hair loss. She experienced in-hospital cardiac arrest due to acute right ventricular failure, likely exacerbated by hyperthyroidism and the administration of amiodarone, which induced thyrotoxicosis. Bedside echocardiography revealed right heart dysfunction, and further tests ruled out pulmonary embolism. The patient was treated with hydrocortisone and methimazole, leading to improved thyroid function and right ventricular recovery. This case highlighted the importance of recognizing thyroid dysfunction in pulmonary hypertension and the utility of bedside ultrasound in diagnosing cardiac arrest causes.
