    Dutasteride: Correct Dosage and Administration

    Dutasteride is a drug that is primarily used to treat benign prostatic hyperplasia (BPH). We have found that its effectiveness lies in the inhibition of 5-alpha reductase, an enzyme that converts testosterone into dihydrotestosterone (DHT), which helps reduce the size of the prostate over time. The same drug, at the same dose, is also used for male pattern hair loss (androgenetic alopecia): it is approved for that use in South Korea, Japan and Taiwan, and prescribed off-label elsewhere. It is a treatment that requires patience, as it can take up to six months to show visible results.

    The approved dose of dutasteride is 0.5 mg once a day, taken as one soft-gelatin capsule. This is the dose on the U.S. FDA label for BPH, and the dose approved in South Korea, Japan and Taiwan for male pattern hair loss. There is no "starting" dose and no higher "maintenance" dose: 0.5 mg once daily is both the first dose and the ongoing dose. Of course, each case is different, so it is essential to follow the doctor's instructions and not make adjustments without consulting them first.

    Dutasteride has a very long half-life, about 5 weeks at steady state, so blood levels build up slowly: roughly 65% of the steady-state level after 1 month and about 90% after 3 months. Because of this, some prescribers use less frequent off-label schedules for hair loss (for example 0.5 mg two or three times a week), and a lower 0.1 mg daily dose is also approved in Japan alongside 0.5 mg. Never take more than the dose your doctor prescribes.

    Administration Instructions

    We have found that the way you take dutasteride influences its effectiveness. Here are some key points to make sure you take it correctly:

    1. It can be taken with or without food, making it easy to incorporate into your daily routine.
    2. It is essential to swallow the capsule whole; you should not chew, open or crush it, as this could affect the absorption of the medication.
    3. Do not take it if the capsule is damaged or leaking, as it can irritate the skin and mucous membranes.
    4. Even if you notice improvement, continue the treatment as directed by your doctor, as stopping it early can cause symptoms to return.

    Scientific Considerations and Precautions

    Throughout our research, we found several important points about dutasteride that you should keep in mind:

    • It is not indicated for women, especially those who are pregnant or of childbearing age, as it can cause malformations in the fetus. - There are studies that have pointed to a possible increase in the risk of developing high-grade prostate cancer, so it is advisable to have regular medical check-ups while taking this medication.
    • It has been observed that it affects prostate-specific antigen (PSA) levels, a key marker in the detection of prostate cancer. If you are undergoing medical check-ups, inform your doctor that you are taking dutasteride.
    • Among the most common side effects we find decreased sexual desire, erectile dysfunction and breast tenderness. In most cases, these effects are usually temporary.

    What to do if you forget a dose?

    Because dutasteride stays in the body for weeks, a single missed dose has very little effect on DHT levels. If one day you forget to take your dose, this is what specialists recommend:

    • If you remember within the same day, take it as soon as you can.
    • If it is already the next day, simply continue with your normal schedule and skip the forgotten dose.
    • Do not take two doses on the same day to make up for the one you forgot, as this will not improve the treatment and could increase the risk of adverse effects.
