TLDR Taking too much vitamin E caused a young man's blood clotting problems, which improved after he stopped the vitamin E and got treatment.
A 31-year-old Indian man developed coagulopathy after taking vitamin E supplements for hair loss, alongside non-steroidal anti-inflammatory drugs for backache. He presented with symptoms of bleeding and was found to have prolonged bleeding and clotting times, indicative of vitamin K deficiency affecting multiple coagulation factors, despite normal serum phylloquinone levels. His serum alpha-tocopherol (vitamin E) levels were marginally elevated. The patient was successfully treated with vitamin K supplementation, pantoprazole, fresh frozen plasma transfusions, and discontinuation of vitamin E supplements. His coagulation parameters normalized, and he remained symptom-free for 6 months post-treatment. This case highlights that even slightly increased levels of vitamin E can inhibit vitamin K-dependent coagulation factors and cause significant bleeding, especially when combined with other bleeding risk factors.
