This report emphasizes the importance of trichoscopy in diagnosing fibrosing alopecia in pattern distribution (FAPD), particularly in differentiating it from female pattern hair loss (FPHL). In a case study of a 49-year-old postmenopausal woman with hair thinning and scalp symptoms, trichoscopy revealed specific features such as areas lacking follicular orifices and hair shaft thickness heterogeneity, leading to a diagnosis of FAPD. Treatment with hydroxychloroquine and finasteride halted disease progression and alleviated symptoms over a 1-year follow-up. Accurate diagnosis is crucial due to the distinct trichoscopic features and differing treatment approaches between FAPD and FPHL.
