Polycystic ovary syndrome (PCOS) is a common endocrine-metabolic disorder with significant health consequences, including infertility. It involves hormonal disturbances such as hyperandrogenemia, insulin resistance (IR), and hyperinsulinemia, which disrupt the hypothalamus-hypophysis-ovary axis. Insulin resistance in ovarian tissue leads to hyperandrogenemia, perpetuating a cycle of IR, hyperinsulinemia, and hyperandrogenemia. Both lean and obese women with PCOS experience chronic inflammation, contributing to cardiometabolic complications like dyslipidemia, metabolic syndrome, type 2 diabetes, and cardiovascular disease. Understanding these pathophysiologic connections is crucial for developing effective treatments and improving the quality of life for women with PCOS.
