Symmetry of Upper Eyelid After Unilateral Blepharoptosis Repair with Minimally Invasive Conjoint Fascial Sheath Suspension Technique

    The study investigated the symmetry of the upper eyelid in 105 patients with unilateral mild and moderate blepharoptosis who underwent minimally invasive conjoint fascial sheath (CFS) suspension surgery. Results showed significant improvements in eyelid symmetry postoperatively, with increased marginal reflex distance 1 (MRD1) and decreased tarsal platform show (TPS) in the ptotic eye, while eyebrow fat span (BFS) remained unchanged. The asymmetry in MRD1 decreased from 1.48±0.86 to 0.58±0.67 (P=0.0004), and TPS asymmetry in mild ptosis patients decreased from 1.15±0.62 to 0.68±0.38 (P=0.0187). The percentage of overlapping curvatures (POC) for eyelid contour symmetry increased from 59.39%±13.45% to 78.29%±13.80%. The study concluded that minimally invasive CFS suspension effectively improves eyelid symmetry in terms of MRD1, POC, and TPS (only in mild cases), with BFS unaffected.
