    TLDR The Bethesda system is effective for identifying thyroid cancer but has low sensitivity.
    This study evaluated the predictive value, sensitivity, and specificity of the EU-TIRADS and Bethesda System classifications for thyroid nodules in 105 patients who underwent thyroid surgery after US-guided FNA. The overall malignancy rate was 45.1%. EU-TIRADS 5 had a sensitivity of 56.90% and specificity of 83.33%, while Bethesda V and VI had a sensitivity of 50.88% and specificity of 95.45%. Both systems showed suboptimal sensitivity but high specificity, particularly the Bethesda system, indicating their effectiveness as clinical-decision tools despite a surgical selective bias in this population.
