Resolution of Waldenström's Macroglobulinemia Related Isolated Neutropenia by Immunochemotherapy

    TLDR Immunochemotherapy successfully treated neutropenia in a patient with Waldenström's macroglobulinemia.
    A 54-year-old male with Waldenström's macroglobulinemia (WM) developed isolated neutropenia, which was resolved through immunochemotherapy with chlorambucil and rituximab. The patient initially presented with slight anemia and elevated CRP levels, progressing to agranulocytosis and recurrent infections. Bone marrow biopsy showed plasmacytoid lymphocyte infiltration and granulopoiesis maturation arrest. Despite initial treatments, including IVIG, failing to improve his condition, the immunochemotherapy led to partial remission of WM, resolution of inflammation, and recovery of neutrophil counts within 4 months. This case highlighted a link between WM and granulocytic maturation arrest, effectively treated with the chosen regimen.
