TLDR Higher PSA levels can indicate prostate cancer, but there's overlap with non-cancerous conditions.
The study aimed to evaluate the diagnostic capability of serum prostate-specific antigen (PSA) levels in detecting prostate cancer, correlating these levels with histological diagnoses. Conducted at the University Hospital of Ioannina, it retrospectively analyzed data from 1,053 patients who underwent prostate biopsies between January 2014 and August 2017. Results indicated that mean PSA values and the percentage of free to total PSA (%fPSA) were significantly higher in patients diagnosed with prostate cancer compared to those with non-cancerous conditions like prostatitis, LGPIN, and HGPIN (p<0.05). Despite this, there was an overlap in individual PSA values among different patient groups. The study found a significant correlation between mean PSA levels and prostate cancer grade (p<0.05). The positive predictive value (PPV) of PSA for patients with levels between 4 and 10 ng/ml was 28%, while the negative predictive value (NPV) for those with levels below 4 ng/ml was 81%.
