In the malignant transition area of a typical proliferating trichilemmal cyst, spindle-shaped cells proliferating in a herring-bone pattern were observed. Keratins were extracted from the tumor and identified using two-dimensional gel electrophoresis, revealing keratins of 56Kd (PI 7.8), 48Kd (PI 5.1), and 46Kd (PI 5.1), which are specific to the outer root sheath. Epidermal keratins were not detected.
