A Rare Case of Ovarian Cellular Fibroma Producing Testosterone

A 62-year-old menopausal woman presented with post-menopausal bleeding, hirsutism, and elevated testosterone levels, which led to the discovery of ovarian cellular fibromas, typically considered hormonally inactive. Her symptoms included worsening facial hair, vaginal spotting, and polycythemia, with blood tests confirming elevated testosterone and hemoglobin levels. Imaging revealed uterine and ovarian abnormalities, and she underwent a total hysterectomy with bilateral salpingo-oophorectomy. Post-surgery, her symptoms resolved, and testosterone and hemoglobin levels normalized. This case highlights the rare occurrence of hormonally active ovarian cellular fibromas, suggesting the need for further exploration of their endocrine activity.
