The study described a novel technique for repositioning lower eyelid fat via a transoral approach in combination with a midface lift, involving six patients followed for at least 1 year. This method improved visualization, reduced operative times, and avoided complications like lower eyelid malposition and retraction associated with traditional approaches. One patient experienced temporary nerve palsy, but no other significant complications were noted. All patients showed improvement in tear trough appearance, lower eyelid herniation, and midface descent. The technique demonstrated potential advantages over standard methods, including technical ease and better outcomes.
