TLDR The congress highlighted concerns about the effectiveness of over-the-counter nail treatments, the link between hair loss and daily care products, and the safety of skin treatments during COVID-19.
The 31st Congress of the European Academy of Dermatology and Venereology (EADV) in September 2022 focused on nail and hair disorders, and treatment safety amid COVID-19. Over 600 speakers presented 170 sessions, with a significant focus on nail diseases. Michela Starace highlighted the limited safety and efficacy data for over-the-counter supplements used for nail disorders. The congress also discussed the increasing prevalence of frontal fibrosing alopecia, a hair disorder causing irreversible hair loss, and its potential association with daily care products. Androgenetic alopecia, the most common hair loss type, was another key topic, with emerging therapies being discussed. Luís Puig emphasized the higher risk of severe COVID-19 in unvaccinated, immunosuppressed patients, but noted that systemic therapies and biologics do not increase this risk. He suggested choosing therapies with longer half-lives to avoid sudden relapses if treatment schedules are disrupted.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.

The conversation is about the role of testosterone in hair loss and the effectiveness of different treatments. The conclusion is that DHT is the main culprit for hair loss, and finasteride has been proven to work long-term in maintaining and regrowing hair. Testosterone may have a minor effect, but it is not the primary cause of hair loss.

Hair loss is linked to cellular physiology and the IGF-1 to TGF-B1 ratio, not just androgen sensitivity. The theory lacks evidence, while finasteride and minoxidil are effective treatments.

Pelage is developing a topical hair follicle stem cell therapy, PP405, for non-scarring alopecias like androgenetic alopecia, with Phase III trials planned and a potential market launch by 2027. The treatment may not require continuous use after initial regrowth.

A user has been experiencing hair loss for 4 years, with treatments like minoxidil, finasteride, and various supplements proving ineffective. They were diagnosed with fibrosing alopecia in a pattern distribution, a condition that may require a combination of anti-inflammatory and hair growth treatments.
