TLDR The patient improved after antiviral treatment for a viral infection.
A 27-year-old renal transplant patient presented with fever, sore throat, and later developed severe dysphagia, vomiting, maculopapular rash, and stomatitis. Initial treatments with antibiotics were ineffective, leading to hospitalization and further investigation. Laboratory tests showed leukocytosis, elevated CRP, and increased serum creatinine, but were negative for various infections. A skin biopsy indicated folliculitis with inflammatory cells. The patient was diagnosed with a viral infection and responded well to appropriate antiviral treatment, resulting in the resolution of symptoms and renal dysfunction.
