Epidermal cyst containing numerous spherules of keratin

The document described a peculiar variant of an epidermal cyst found in a 56-year-old man, which differed from a typical epidermal cyst. While one of the man's subcutaneous nodules was an ordinary epidermal cyst, the other exhibited unique characteristics. It was soft, with a brownish, lumpy content resembling bone marrow. Microscopically, the cyst was lined with epidermis containing granular cells and orthokeratinization, and its contents included numerous keratin spherules. These spherules formed just above the keratinous layer of the lining epithelium, and there were shadow cell-like foci with mild mineralization observed.
