TLDR Doxazosin alone or combined with finasteride improves prostate symptoms better than finasteride alone, without extra benefits.
The PREDICT study compared the efficacy and tolerability of doxazosin and finasteride, alone and combined, for the symptomatic treatment of benign prostatic hyperplasia (BPH). The study involved 1095 men aged 50-80 years, treated for 52 weeks in 90 European centers. The results showed that treatment with doxazosin alone or combined with finasteride gave statistically significantly better International Prostate Symptom Score (IPSS) and maximum urinary flow rates (Qmax) than did placebo and finasteride alone. Adding finasteride did not provide further benefit to that with doxazosin alone. The symptomatic improvement appeared to be independent of prostate size and PSA level. The study also found that the incidence of impotence with doxazosin and combination therapy was similar to that reported with terazosin and a finasteride plus terazosin combination.
