    A 32-year-old HIV-positive man presented with dizziness, weight loss, unsteady walking, loss of eyebrow hair, and an itchy rash. Examination revealed an ataxic gait, a generalized scaly maculopapular rash, and ulcers on the hard palate. The most likely diagnosis was secondary syphilis with neurological involvement, as other conditions like guttate psoriasis, mycosis fungoides, and leprosy were less likely to cause the observed neurological symptoms and systemic illness.
