Dermatophytic Granuloma Caused by Microsporum Canis in a Heart-Lung Recipient

A 66-year-old heart-lung transplant recipient developed dermatophytic granuloma caused by Microsporum canis, presenting as erythematous pustules and papules on the forearm. Histological examination and skin biopsy cultures confirmed the diagnosis. This case highlighted the risk of deep dermatophytic infections in immunocompromised patients, with only 4 other similar reports. The infection likely progressed from hair follicles to the dermis. Topical antifungal therapy was ineffective, and fluconazole was identified as the preferred treatment.
