A Cost-Utility Analysis Comparing Oncoplastic Breast Surgery with Standard Lumpectomy in Large-Breasted Women

    TLDR Oncoplastic breast surgery is more cost-effective than standard lumpectomy for large-breasted women despite being more expensive.
    The study conducted a cost-utility analysis to compare oncoplastic breast surgery (OBS) with standard lumpectomy (SL) in large-breasted women undergoing treatment for unilateral breast cancer. The analysis included a systematic literature review, utility score surveys, Medicare payment data, and decision analysis tree and incremental cost-utility ratio analysis. The results showed that OBS led to fewer positive margins (10% vs 18%) and higher utility scores for successful operations (92.6 vs 86.55) compared to SL. However, OBS was more expensive ($6782.36 vs $2399.99). Despite the higher cost, OBS was found to be more cost-effective with an incremental cost-utility ratio of $2473.54 per quality-adjusted life year. Sensitivity analysis indicated that SL would only become more cost-effective if the utility score for successful surgery was greater than 92.33. The conclusion was that OBS provides a cost-effective treatment option for large-breasted patients and should be considered as a primary treatment option.
