    TLDR Eyelash transplantation can cause eye damage and pain if not done carefully.
    A 31-year-old female with generalized alopecia underwent eyelash transplantation using follicles from the nape of her neck, resulting in trichiasis and corneal punctate epitheliopathy. Initially satisfied, she experienced severe ocular pain and lid swelling within 5 days. Examination revealed misdirected eyelashes causing corneal damage. Despite initial treatment with chloramphenicol eye ointment and lubricants, symptoms persisted until the misdirected eyelashes were epilated, leading to rapid healing and improved visual acuity. This case highlighted the potential for ocular complications from eyelash transplantation, emphasizing the need for thorough preoperative counseling.
